If books are included with your course, then you must first fill out the book voucher form below. If this form is not filled out your books will not ship. Books ship UPS and will be sent out within 3-5 days after the student registers.
Student book voucher Information:
Acknowledgement. This is a legally binding contract between the Student and the Institution Phlebotomy Career Training 28050 Ford Rd. Ste C Garden City, MI.. The Student understands that they may cancel this Contract within 24 hours, no later than midnight from the time that the Student signs this Contract, by sending the Notice of Cancellation or other written notice to the
Institution on or before that date. *There will be absolutely no refunds after the start of the first day of a program,or for online students, the day that they access the course, regardless of refund/cancellation period.
Tuition Refund/Cancellation Policy. An application fee of not more than $150.00 will be retained by the Institution upon students request for refund within the allotted 24 hour period. Furthermore, student acknowledges that they are responsible for the entire cost of the class if they fail to make the payments as agreed upon during their registration process and are dismissed
due to non payment.
